Waivers and Consent Forms:
Registration Requirements
- Families must be registered parish members to enroll children in Faith Formation.
- Submit:
- Faith Formation program registration
- Image Release Form
- Parental Opt-Out Form
- Consent Form for Electronic Communication
- Annual Registration Fee
- Payment Options: The fee may be paid in installments throughout the year.
- Financial Assistance: No child will be denied participation due to financial hardship.
Required Documents
Please bring copies of the following documents at the time of registration:
- Child’s Birth Certificate
- Child’s Baptism Certificate (if received)
- Child’s First Holy Communion Certificate (if received)
Sacramental Preparation:
First Reconciliation & First Holy Communion
- Typically offered to students in 2nd Grade and above with a previous year of faith formation
- Parents must attend scheduled parent meetings
- Students must attend all classes and additional preparation sessions
Confirmation
- Offered to students in 8th Grade and above with recent year of faith formation
- Classes are held usually on the first Sunday of each month from 1:00pm-4:00pm
- Youth Group meetings and additional preparation sessions are required
- Parents and sponsors must attend designated sessions prior to the Confirmation date

Classroom Conduct
We strive to maintain a respectful and engaging learning environment. If a student exhibits disruptive behavior:
- The catechist will notify the Faith Formation Director.
- If the behavior continues, the Faith Formation Director will inform the parents.
- Parents may be asked to pick up their child and attendance requirements may be affected.
